Objective:
Studies are proposed to identify health hazards of source-specific physicochemical components of fine PM (e.g., UFP; organics) in epidemiological, controlled clinical, animal, and in vitro studies; a main focus will be on characterizing fine PM and determining pathophysiological mechanisms of effects to test the hypothesis that ambient ultrafine (UF)/fine PM cause oxidative stress and trigger cardiovascular adverse health effects, with specific emphasis on events leading to endothelial dysfunction.
Progress Summary:
Core 1: Exposure Assessment and PM Source Identification
During this year, work has continued to improve our understanding of the ambient aerosol through the development of on-the-fly particle source identification and apportionment and the identification of new chemical components that contribute to the particle-bound reactive oxygen species in the ambient aerosol. These results will lead to new approaches for providing key exposure assessment to support the toxicology and clinical studies in the Center. In addition, we have provided other critical data to other research cores. For example, particle size distribution and other pollutant concentration data support the clinical studies of heart rate variability and inflammatory markers in the blood of rehabilitation patients. We will be using our existing particle size distribution data base to initiate an epidemiological study of particle number concentrations and mortality in Rochester in conjunction with the Harvard PM and Health Center, and we are working with the in vitro core to analyze the results of their composition response data to see if we can identify relationships between composition and response.
Core 2: Epidemiological studies on extra pulmonary effects of fresh and aged urban aerosols from different sources
The objective of the epidemiological study is to determine the effect of fine and ultrafine particles on various blood markers. It is aimed to enroll 240 participants in three panels consisting of subjects with (1) type 2 diabetes, (2) impaired glucose tolerance (IGT) and (3) potential genetic susceptibility. Particles are measured at a central measurement site in Augsburg. In addition, personal measurements of temperature, humidity, noise and ultrafine particles are undertaken in a subset of 90 participants from all three panels. For this subset an endothelial function test and a 4-hour ECG are also scheduled.
A pilot phase was conducted between 19th and 23rd of February 2007. No major changes in the study protocol seemed necessary. The main phase started on the 19th of March and the recruitment is still ongoing. Up to the 25th of May 45 participants had been recruited by telephone (13 diabetics, 13 with IGT and 19 genetically susceptible participants). Sixteen agreed to take part in the personal measurements (nine diabetics, six with IGT and one of the genetically susceptible participants). In the first ten weeks of the field phase 73 examinations have been conducted with a maximum of three visits per participant. Data collection will proceed until the target of 80 participants per panel is reached.
As presented at the EPA-GSF-Rochester meeting in Montreat, NC, in spring 2007, first results from the pilot study indicate a good relation between personal exposure and the data of the central measurement site on an hourly basis
Core 3: Human Clinical Studies of Concentrated Ambient Ultrafine and Fine Particles
The overall Core 3 hypothesis is that inhalation of ambient ultrafine and fine PM causes small but measurable changes in cardiac and vascular function, and coagulation, that help explain the cardiovascular effects of PM exposure. We are conducting a clinical study of healthy subjects inhaling concentrated ambient UFP. This study is co-funded by the EPA and by an NIEHS RO1 grant. Nine of a projected twenty subjects are enrolled in the study and six have completed both exposures. Exposures have been successful from a technical standpoint and no subject has experienced symptoms or problems.
We have completed a study of type 2 diabetics, age 30 to 60, inhaling filtered air and 50 μg/m3 carbon UFP for two hours, at rest. Nineteen subjects completed this study. We found evidence that UFP exposure increased markers of platelet activation, and also increased the number of circulating microparticles expressing tissue factor. These findings indicate that UFP exposure transiently activated vascular endothelium and/or platelets in people with type 2 diabetes. This supports the hypothesis that exposure to ambient UFP increases the potential for vascular thrombosis in patients with severe vascular disease.
Using in vitro studies, we have found that carbon ultrafine particles have direct effects on platelet activation. The degree of activation varied with particle size and composition. Thus one mechanism for UFP effects may be direct effects on platelets from particles that enter the circulation.
Core 4: Animal Models: Cardiovascular Disease, CNS Injury and Ultrafine Particle Biokinetics
The objective of the Core 4 studies is to correlate physicochemical particle characteristics with pulmonary and cardiovascular endpoints following exposures to ultrafine particle (UFP)-containing ambient aerosols in a rat model of type II diabetes obese, (hyperinsulinemic JCR rats). Effects measurements are coordinated with the epidemiological, clinical, and in vitro studies.
The diabetic rat model (male JCR rats) was used to study pulmonary and cardiovascular endpoints following exposure to either ambient concentrated ultrafine particles or to diesel exhaust while driving on the highway. Non-obese, nonhyperinsulinemic rats (JCR +/?) not developing atherosclerotic lesions served as controls.
After exposure for 4 weeks (6 hrs./day; 5 days/week) to concentrated Rochester ultrafine particles (count median diameter 75 nm; number concentration 0.05 – 1.3 x 106 particles/cm3) changes in the following endpoints were observed in the obese rats compared to the normal lean rats: increase in total lung lavage cell number; lavage fluid protein concentration and lavage LDH and β-glucuronidase. Blood parameters: total white blood cell number; number of blood leukocyte aggregates; and plasma fibrinogen. These changes were observed after 3 days of exposure, but not after 4 weeks of exposure, indicating an adaptive response. Likewise, after 3 days of exposure aortic mtDNA amplification was blunted, suggesting that oxidative damage had occurred. This response was not seen after 4 weeks of exposure. However, it appears that iron homeostasis was disrupted in the obese rats following 4 weeks of exposure: higher levels of iron were measured in lung lavage fluid, liver and lung tissue, as well as higher levels of transferrin and ferritin in the lavage fluid. In addition, serum and lavage fluid levels of leptin were increased in the obese rats after 4 weeks of exposure (indication of defense mechanism induction?).
The on-road exposures were designed to compare respiratory and cardiovascular effects of inhaled diesel exhaust particles from low (pre-2007) and ultralow (2007) diesel fuel, simulating real-world conditions while driving the rats on the New York State Thruway between Rochester and Buffalo for 6 hours each day in the Mobile Exposure Laboratory. Rats were implanted with radiotransmitters to continuously monitor heart rate, blood pressure, body temperature and activity. Evaluations of blood pressure, heart rate and heart rate variability are on-going. Although the lung lavage of obese rats had higher total cell numbers, PMN, as well as higher protein, LDH and β-glucuronidase levels than the lean rats, exposures to the exhaust for either 1 or 4 days did not have any consistent effects. Other endpoints related to inflammation using bead array technology did not reveal significant changes, although there was a lot of variability, with the obese rats generally having higher levels. The only exposure-induced effect was in serum leptin levels which were increased in the exhaust-exposed obese rats. Lastly, as we have found previously, UFP-containing aerosols caused a decrease in heart rate; the differences between exposure groups got larger over time.
Core 5: Ultrafine particle cell interactions in vitro: Molecular mechanisms leading to altered gene expression in relation to particle composition
The experiments proposed within this project address specific mechanistic hypotheses regarding the interactions between inhaled ultrafine particles and specific pulmonary cell populations. They attempt to provide a mechanistic link and biological plausibility for the whole animal and controlled clinical (human) exposures, described in the other programs of this Particle Center. During the current year we have focused on better characterizing the response of vascular endothelial cells to PM, as this is in keeping with our effort to understand the cardiovascular effects. We established appropriate culture conditions for the microvessel endothelial cells and compared the PM response to that of HUVEC cells.
We determined that production of IL-6 and nitric oxide (NO) were useful markers of PM response and established conditions to measure them. We also showed that cells cultured in elevated glucose demonstrated a blunted response to PM consistent with altered oxidative stress.
We also compared the production of NO and IL-6 in cells exposed to PM of varying composition collected at various sites in the US with a high volume sampler. We are attempting to correlate the cellular response to the size and composition of the PM. This remains a work in progress.
