Objective:
The overarching objective of the Center was to link inflammation, autonomic dysfunction and vascular dysfunction to specific particulate matter (PM) characteristics and sources. In addition, Center research addressed important scientific questions such as: i) What types of pathophysiological effects are produced by PM exposures and how do these effects relate to specific PM properties and sources (toxic components); ii) What is the effect of gaseous co-pollutants on the observed PM exposure-response relationships; iii) Which are the underlying biological mechanisms whereby PM exposures can induce inflammation, autonomic dysfunction and/or vascular dysfunction; and iv) Are certain individuals more susceptible to PM due to their health condition, age, genetic characteristics and nutrition habits.
Summary/Accomplishments (Outputs/Outcomes):
The Center encompassed four highly interdisciplinary and integrated projects designed to address the four scientific questions presented above. Project 1 investigated the health effects of PM in the Normative Aging Study cohort, in Eastern Massachusetts; Project 2 examined the health effects of fine and coarse CAPs in healthy adults, in Toronto; Project 3 investigated the health effects of locally emitted and transported PM in Sprague Dawley rats and spontaneously hypersensitive rats, which were exposed to fine criteria air pollutants (CAPs) in Boston; and Project 4 investigated the health effects of primary and secondary vehicular emissions from a tunnel in the northeastern United States using healthy and spontaneous hypertensive rats (SH). Finally, the above four proposed projects were supported by three Cores: the Administrative, Particle Technology and Monitoring, and Statistical Cores.
Conclusions:
Major Findings: To date 100 peer-reviewed papers have been published as a result of the research conducted by the four projects and two cores (see list of publications). This final report highlights most of the Center findings for each project. Below we present a brief summary of these findings.
Project 1: Multi-pollutant air pollution effects were investigated by examining modifications of the health effects of PM2.5 mass by various air pollutant species, gene polymorphisms (GPM), and air mass back trajectories (AMBT). The following relations were observed: 1) lead, SO42-, and O3 effects on heart rate variability (HRV); 2) GPM protection against HRV effects of PM; 3) transition metal effects modified by GPM; 4) black carbon (BC) and ozone effects on HRV modified by AMBT, and; 5) PM2.5 species effects on low frequency (LF)/high frequency (HF) ratio modified by AMBT.
Address-specific BC values, determined by the GIS-based spatial smoothing model that was developed at Harvard School of Public Health (HSPH), were used to show an inverse association of BC with telomere length. Other associations that were found between traffic related PM and/or PM2.5 include: 1) inflammatory markers; 2) elevated plasma total homocysteine; 3) effects modification by genes related to oxidative stress, and; 4) increased vascular cell adhesion, with modification by obesity and genes. Both PM2.5 and SO42-, but not BC were associated with oxidative stress-related urinary 8-OHdG.
Association was found between PM2.5 and decreased HRV. These effects were modified by genetic polymorphism in endothelial and methyl-related genes, as well as by dietary intakes levels of methyl nutrients. Obesity was a significant factor in the susceptibility to health impacts of ozone and traffic-related PM.
Project 2: Induced sputum showed a greater inflammatory response for coarse CAPs compared to fine CAPs exposures, as evidenced by a larger number of airway neutrophils 24 hours after coarse particle exposures. The observed differences were not related to exposure mass concentration as the target CAPs level was lower for the coarse vs fine CAPs (200 vs 250 µg/m3). A more robust association between β-(1,3)-D-glucan concentration and the number of sputum neutrophils for coarse vs. fine CAPs was observed, suggesting that the biologic component of PM may be important. Some interesting and exciting findings were observed in a subgroup of subjects in whom the epigenetic effects (DNA methylation) of coarse and fine CAPs exposures were observed. Fine particle exposures induced Alu and IL-12 hypomethylation, while coarse particles, endotoxin and β-(1,3)-D-glucan induced TLR-4 hypomethylation. Furthermore, these acute DNA methylation changes might be on the pathway to increased systemic white blood cells (WBCs) and elevated blood pressure.
In addition to the coarse and fine CAPs project, we observed some key findings from collaborative retrospective analyses of our controlled fine CAPs and O3 exposures in healthy subjects at rest. First, the importance of considering prior exposure to ambient pollutants (i.e., O3, PM2.5, SO2, NO2) on baseline measures prior to chamber exposures was highlighted by the observed positive associations with the inflammatory marker IL-6. Second, we observed a dose-response relationship between CAPs mass concentration and HRV measures of autonomic tone that varied depending on the addition of O3, again highlighting the importance of examining multi-pollutant exposures. Third, we observed a significant increase in cardiac repolarization time after exposure to CAP+O3; this finding supports epidemiological studies reporting sudden or arrhythmic events associated with air pollution exposures.
Overall, our findings provide important insights into the mechanisms of response to inhaled particles, highlighting the importance of considering particle size, composition/sources and multi-pollutant mixtures on respiratory and cardiovascular pathways.
Project 3: Overall, the findings were consistent with our earlier CAPs exposures studies in Boston. In general, although there were statistically significant differences in CAPs composition between early morning and mid-day exposures, the differences were modest. As expected, the differences in composition were consistent with increased contribution of local sources in the early morning.
In normal rats, both the early morning and mid-day CAPs exposures generated significant responses in several respiratory parameters and in vivo chemiluminescence of the lung. Circadian differences in respiratory parameters, which were significant in animals exposed to filtered air, were controlled for in analysis. The respiratory responses, featuring a rapid shallow breathing pattern, were consistent with an irritative response to exposure. In SH rats and their strain control (WKY) rats there was a significant circadian blood pressure response in exposure to filtered air in the early morning compared to mid-day. In the SH rats, the blood pressure levels were sufficiently high that they may have been beyond influence by CAPs exposure. However, in the WKY strain controls, when controlling for circadian effects, there was a significant response to CAPs exposure that was associated with elements related to road dust composition.
Although there are modest statistically significant differences in the composition of early and late exposures on given days, there is no significant difference in toxicity. Our studies have adequate statistical power to detect significant diurnal differences with respiratory patterns, significant strain differences, as well as CAPs vs Sham differences. Because both early morning and mid-day exposures show significant toxicity, with no significant difference in the biological outcomes between the two exposure periods, these results suggest that either (1) the modest composition differences between morning and mid-day exposure are not sufficient to generate different biological outcomes, or (2) there is little difference in the toxic potential of local and transported sources.
Project 4: Results from the Power Plant TERESA study informed us that it is very difficult to assess the comparative toxicity of particles in different exposure types that have widely different concentrations. For this reason, the mobile source TERESA study conducted exposures to primary (P), aged primary plus secondary (P+SOA), and secondary aerosols (SOA) at consistent concentrations.
All of the exposures produced respiratory changes compared to filtered air, but more prominent respiratory toxicity was found in animals exposed to P+SOA. Overall, P+SOA>P>SOA for respiratory outcomes. The effect was greater in animals with repeated exposures. Significant breathing pattern changes were associated with the different exposure types. These changes were internally consistent, with patterns that indicate an irritative response to exposure. This response is mediated by local (respiratory effort) and central (respiratory drive) components and is augmented with repeated exposure to fresh and aged vehicular emissions. Four-day exposures to P+SOA produced the most significant and complex response in breathing pattern, with decreases in volumes, flows, respiratory drive and respiratory effort. This suggests that photochemical oxidation and formation of secondary particle mass enhanced the respiratory toxicity of primary traffic derived particles.
Both primary and secondary traffic related aerosols can substantially increase systolic blood pressure (SBP) and diastolic blood pressure (DBP). Initial increases up to 15 mmHg in DBP were recorded for the P exposure. This is a greater blood pressure change than has previously been reported in the literature in animal or human studies for either similar or substantially higher PM2.5 concentrations. The responses are sustained with repeated primary aerosol exposures, but not with secondary aerosols. Animals exposed to secondary aerosols showed a gradual decrease in the magnitude of response with repeated exposures, and subsequently show compensatory decreases in both SBP and DBP after the 3-week protocol. This unexpected but significant compensatory response suggests a biological protective effect with repeated exposures that cannot be explained by simple autonomic nervous system activation. These results confirm the adverse health effects associated with inhalation of fine particles in previous studies and give insight into a potential biological adaptation to maintain blood pressure in response to repeated and anticipated pollutant exposure. Greater limitation in the BP response in animals exposed to secondary particles requires further investigation to define the interaction of this exposure with blood pressure control mechanisms.
