Objective:
Objective of Research Project 1: To build upon our discovery of immunologic and molecular biomarkers specific to children with autism found in 2-5 year olds enrolled in the CHARGE (Childhood Autism Risks from Genetics and Environment) Study.
Objective of Research Project 2: Our current aims include the continued examination of environmental toxicants such as PBDE47 on immune function in children with autism. In addition, we are currently studying the profile of autoantibodies in both the maternal population and their offspring. We will bring back our subjects for a longitudinal analysis of their immune function parameters studied previously. We are currently waiting for IRB approval to contact our extension subjects. During this time, we are perfecting our B cell functional analysis using adult PBMC. All aspects of this system are currently working well.
Objective of Research Project 3: The specific aims of Project 3, “Models of Neurodevelopmental Susceptibility,” remain unchanged. The overall goal of Project 3 is to determine if exposure to environmental toxicants early in development contributes to the etiology of neurodevelopmental disorders. The strategy is to expose mice perinatally to environmental agents (i.e., BDE 47, PCB 95 and MeHg), and to discover and characterize possible neurotoxicity of exposure on brain development, neurobehavioral performance and immune system function. Progress to date is described below:
Progress Summary:
Project 1
Newborn blood spots were obtained for 483 CHARGE study children. These bloodspots were sent to the laboratory of Dr. Ron Torres (Utah State University) for processing and are now back at UC Davis and ready for analysis of metals and cytokines.
Metal concentrations are being compared across developmental groups. The final batches of whole blood covering all children enrolled in the CHARGE Study during the first 5-year funding period of the CCEH have been analyzed for metals, including Hg, Pb and As. The total number of children with metal analyses completed is now 624. A manuscript is in preparation comparing Hg concentrations across developmental case groups, and examining the sources of blood Hg.
Protocols and recruitment priorities for the CHARGE-BACK study have been developed. This sub-study will evaluate the immunologic profiles of children who participated in the CHARGE Study 1 or more years after the initial assessment, to determine whether the lower responses among children with autism persist over time. Of particular interest are the reduced concentrations of IgG and IgM, as well as the T-cell responses to bacterial and viral antigens. To select the 375 children for this sub-study, first priority will be those in the highest and lowest quartiles of IgG or IgM. The protocol also includes updating information on the child’s sleep habits, the family auto-immune disorders, and the services and treatments the child has received. The blood draw form also will collect information about medications and food intake in the previous period.
The MARBLES (Markers of Autism Risk in Babies – Learning Early Signs) Study, a longitudinal cohort study of pregnancies of high-risk children was launched, after a pilot phase. The pilot phase began recruitment in July 2006, supported by funding from the M.I.N.D. Institute. A total of six women were enrolled by the end of December. During this time, we focused on creation of the infrastructure necessary to complete the study. Components included: recruitment script; maternal diary; specimen collection protocols for blood, urine, hair, cord blood, placenta, breast milk, and dust; electronic recruitment and subject tracking systems; staff training to organize and carry out home visits and attend subject births; and transport, processing and archival of specimens according to protocol specifications.
To date, we have enrolled 13 women, conducted more than 30 home visits, attended 4 deliveries and obtained specimens from a fifth. We have collected 20 first and second trimester maternal blood samples, 10 maternal urine samples, and 11 hair samples. For the third trimester specimens, we have an additional 8 maternal blood, 7 maternal urine, and 2 maternal hair samples. Prenatal home dust collections have been completed for 9 subjects. Postnatal specimen collections include five 2-week baby urine (diaper) samples and one 3-month urine sample, one 3-month baby blood sample, 4 baby hair samples, and 5 breast milk collections. Additionally, 100 maternal symptom diaries have been completed.
We have received support from the UC Davis General Clinical Research Center (GCRC) in the Clinical Translational Science Center, which now provides staff to attend home visits and carry out maternal and pediatric phlebotomy services for our study subjects. Procedures related to environmental sampling have been refined and dust samples have been collected at home visits.
We have recruited roughly equal numbers of women drawn from outreach efforts to local parents’ groups, families who have participated in our CHARGE Study during the first funding period of the CCEH, and referrals from other studies conducted at the M.I.N.D. Institute. State of California IRB approval was recently obtained to contact families of children eligible for services funded by the California Department of Developmental Services.
In collaboration with the Community Outreach and Translation Core (Core 2 of the CCEH), we have designed and implemented the initial stages of a comprehensive outreach effort targeting families through autism support groups, obstetric and pediatric providers, and providers of services for children with developmental disabilities.
We have launched the first longitudinal study of autism examining environmental and molecular factors beginning in pregnancy (MARBLES). Determination of whether immunologic differences between children with autism and other children are persistent will shed light on whether immune dysfunction is incidental or integral to this complex disorder (CHARGE-BACK). Similarly, determination of metal concentrations at birth will help clarify whether Hg exposures in early life are associated with autism risk (blood-spot study).
The CHARGE Study, which was launched in the first 5-year period of the CCEH, will continue for 5 more years as a separately funded R01 (The CHARGE Study: Childhood Autism Risk from Genetics and the Environment, R01-ES015359, Hertz-Picciotto PI). By 2011, the study will include 1,600 families, the largest and most comprehensive study of environmental factors in autism.
An application to the NIH for a multi-site study modeled after the MARLBES Study was submitted in the form of an Autism Center of Excellence (ACE) Network. This project, “Early Autism Risk Longitudinal Investigation” or EARLI, will be funded in 2008 (1 R01-HD055803), with Dr. Craig Newschaefer at Drexel University as PI. UC Davis will be the vanguard site of this study, and will house the Data Coordinating Center.
Project 2
(1) To examine the effect of in vitro xenobiotic exposure such as PBDE47 on peripheral blood mononuclear cell (PBMC) function. We have noted a change in the response of PBMCs from subjects with autism compared to controls when in the presence of PBDE47. There is a profound decrease in the response to LPS by the monocyte/macrophage population in the AU population. This suggests that they are extremely sensitive to such exposure and this could ultimately affect immune function.

(2) The presence of these antibodies in the plasma of some mothers of children with autism, as well as the differential findings between mothers of children with early onset and regressive autism may suggest an association between the transfer of IgG autoantibodies during early neurodevelopment and the risk of developing of autism in some children.

(3) We have noted reduced levels of plasma IgG and IgM in the AU population compared to both DD and TD controls suggestive of an underlying defect in immune function. This reduction in specific Ig levels correlates with behavioral severity, where those patients with the highest scores in the behavioral battery have the most reduced levels of IgG and IgM.

We have noted several significant findings during the past year. The first is the presence of maternal autoantibodies in a subset of subjects that could potentially be used as a diagnostic marker for autism risk. The second is the relationship between behavior and immune dysfunction as noted by the correlation between IgG and ABC score. This suggests to us that there is a strong link between immune dysfunction and severity of autism. Finally, we have preliminary studies that suggest the individuals with autism are more sensitive to PBDE 47 with respect to direct effects on immune function.
Project 3
A comprehensive battery of neurobehavioral tests, including measures of social behaviors, memory and learning, sensory and motor development, sensory gating and anxiety was used to assess in C57BL/6J mice neurodevelopmental toxicity of BDE-47, PCB 95 and MeHg (Specific Aim 1). Initial studies have used perinatal exposure of C57BL/6 mice to BDE-47 at concentrations of 0.03, 0.10 and 1.0 mg/kg/day. Litters were divided into subgroups, with male and female mice in each subgroup used for neurobehavioral studies, assessment of immune system function, or neurohistology, respectively. Behavioral testing of the first cohort of 70 mice exposure to BDE-47 is completed, and a second cohort is undergoing exposure to BDE 47 to increase numbers of mice in each treatment group. Immune system function is also being analyzed in subsets of mice in the first cohort, and brains of mice used for behavioral studies were perfused and prepared for histological analysis. A detailed structure-activity study of PCB-sensitized ryanodine receptors (RyRs) was published this project period (Pessah et. al 2006; Ta and Pessah 2007). Behavioral and neurophysiological experiments with perinatal and lactational exposure to PCB-95 and PCB170 in the Sprague Dawley rat have shown an enhanced seizure susceptibility and excitotoxicity that is synergized by depression of GABAergic neurotransmission. One manuscript has recently appeared in press (Kenet et. al 2007) and another is nearly ready for submission for peer review (Kim et. al). More extensive behavioral studies in C57BL/6 mice are nearing completion and these data will undergo statistical analysis during the coming months. A detailed study on how neonatal thimerosal and vaccine exposures influence neurobehavior and development of the hippocampus has been submitted for peer review (Berman et. al 2007).
Specific Aim 2 examines the effects of perinatal exposure of mice to MeHg, PCB 95 or BDE-47 on immune system function. B-cell proliferation was studied in splenocytes from the offspring of BDE-47-exposed mice (0.03 or 0.10 mg/kg/day versus untreated controls and vehicle controls) using lipopolysaccharide (LPS) and phytohaemagglutinin (PHA). The pattern of preliminary data suggests that perinatal exposure to low levels of BDE-47 (i.e., 0.03 and 0.1 mg/kg/day) may have enhanced mitogen-stimulated proliferation of both T-cell and B-cell/macrophage monocytes (Figure 4). These studies will continue, as well as serological characterization of immunoglobulin and cytokine levels in mouse offspring exposed to BDE-47.
In collaboration with Core 3, we have also determined tissue levels of BDE-47 by GC-MS (Specific Aim 3). Brain, blood, liver, muscle and adipose tissue levels of BDE-47 have been measured following perinatal (i.e., combined prenatal and postnatal) exposure to 1 mg/kg/day, and ongoing experiments are examining tissue levels following lower exposure levels (0.03 and 0.1 mg/kg/day). Results thus far demonstrate that at an exposure level of 1 mg/kg/day there is a marked accumulation of BDE-47 in all tissues examined, including brain and blood (Figure 5). Furthermore, offspring of dams given BDE-47 through gestation and preweaning show tissue levels that are similar to those found in the dams, demonstrating transfer of BDE-47 to the mouse fetus.

Specific Aim 4 examines the effects of prenatal exposure of pregnant mice to immunoglobulin G (IgG) isolated from biological mothers of autistic children on brain development and behavior. The hypothesis is that perinatal exposure to autoantibodies to fetal brain proteins in purified IgG isolated from mothers of autistic children (i.e., IgG-A) will alter brain development, impair behavior and increase susceptibility to methyl mercury compared to IgG-C from mothers of control children in the general population.. We have carried out two initial IgG studies in C57BL/6 mice. These studies used 1 or 2 injections of IgG-A or IgG-C given on gestational days 12 and 16. Two injections were found to result in a high mortality rate in the offspring of both groups, while a single injection did not appear to increase mortality. This suggests that an immune complex reaction to the second injection of purified human IgG may have occurred. As a result we are currently repeating these experiments using a single IgG-A or IgG-C injection on gestational day 12, and will carry out behavioral, histological and immunological studies as proposed. Blood samples from the initial IgG-injected dams and offspring have been collected and will be analyzed for the presence of mouse antibodies directed towards human IgG to determine if an immune reaction occurred in either the dams or pups.
These studies are significant because they: (1) examine the potential for environmental toxins to contribute to the risk for neurodevelopmental disorders such as autism, (2) explore the interactions between exposure to environmental toxins and immune system dysfunction, and (3) are focused on the effects of chronic low-dose exposure. If early exposure to environmental agents can be shown to be a risk factor for neurodevelopmental disorders, then regulatory controls can be put in place to reduce the risk. In addition, such findings could help explain the apparent dramatic increase in the incidence of neurodevelopmental disorders that is occurring world wide.

Future Activities Project 1:
Recruitment for the MARBLES study will be intensified, and protocols will be finalized for the 6-, 12- and 24-month visits. CHARGE-BACK recruitment will begin. Manuscripts will be finalized and submitted on metals and PBDEs. Analyses of blood-spots will be conducted.
Future Activities Project 2:
We will expand our PBDE 47 findings to increase our data points and determine the correlation between these data and that found in the mouse exposure studies with Project 3. We are currently working on the identification of the maternal antibody target proteins in the hope of developing a screening assay. Finally, we will examine the cellular mechanism responsible for the low IgG and IgM production.
Future Activities Project 3:
The plans for the next grant year are to: (1) complete the BDE-47 and PCB 95 experiments in C57BL/6 mice by increasing the number of animals used in the neurotoxicity studies; (2) carry out the neurohistological examination of toxin-exposed mice; (3) complete the IgG experiments using the revised, one injection protocol; and (4) complete the B-cell mitogen-stimulated cell proliferation studies and the cytokine measurements in BDE-47 and PCB 95 exposed mice.
