Objective:
The objectives of the Children’s Environmental Health Center are to: (1) test the efficacy of an intervention for the primary prevention of lead toxicity, as measured by lead concentration and neurobehavioral functioning at 36 months of age; (2) serve as a model to evaluate the adverse effects of exposures to multiple prevalent toxicants among fetuses and children; (3) test meconium levels as a biomarker of fetal exposure to numerous toxicants; and (4) provide exposure and risk assessment data for residential pesticides.
Exposure to numerous environmental agents, including lead, mercury, polychlorinated biphenyls(PCBs) , and environmental tobacco smoke (ETS), has been linked with adverse neurobehavioral effects. Still, the ideal biomarker for measuring in utero exposure to specific toxicants has not been established and the adverse effects of many potential neurotoxicants have not been tested rigorously. Fetal exposure typically is measured with self-reported surveys, maternal blood and urine, or cord blood. In contrast, meconium as a biomarker is a noninvasive method to test simultaneously for cumulative exposures to numerous toxicants. It is unclear, however, whether conventional biomarkers or meconium levels are more predictive of the adverse effects linked with specific toxicants. For lead exposure, emerging data indicate that our efforts should emphasize primary prevention; however, the efficacy of lead hazard controls is uncertain, especially for children with low blood lead concentrations.
We propose to conduct a cohort study of 400 children, followed from less than 16 weeks’ gestation to 36 months of age, to examine the effect of low-level exposures to prevalent neurotoxicants. Endpoints include behavioral problems, such as conduct disorder and features consistent with Attention Deficit Hyperactivity Disorder, cognitive deficits, and hearing loss. We also will conduct a nested, randomized controlled trial to test the efficacy of lead hazard controls on the development of adverse neurobehavioral effects. We will test the following hypotheses: (1) children in the Lead Reduction Group will have lower blood lead levels, significantly higher cognitive scores, less hearing loss, and fewer behavioral problems than the Control Group at 36 months of age; (2) fetal and postnatal exposures to pesticides, ETS, and lead are associated with adverse neurobehavioral effects, growth delay, and hearing loss in early childhood; and (3) fetal exposures are less predictive of the adverse effects of toxicants on cognition, behavioral problems, and hearing, compared with the same toxicants in meconium.
Progress Summary:
Cincinnati Children’s Environmental Health Center (CEHC) has had a productive year. The administrative activities of the Center are described in this progress report. In our last progress report, we described a meeting, held in Cincinnati in May 2003, of principal investigators representing longitudinal lead-exposed cohorts from the United States, Australia, and Mexico. This meeting was arranged to study the results of the analyses of pooled data from more than 1,300 children and the findings of intellectual impairments at blood lead levels below 10 μg/dL, compared with higher blood lead levels. Our analyses have been completed, and a manuscript was submitted recently to the Journal of the American Medical Association. The title of the paper is “Low-Level Environmental Lead Exposure and Children’s Intellectual Function: An International Pooled Analysis.”
As mentioned in our previous progress report, National Public Radio’s weekly environmental science show, “Living on Earth,” received a grant from the National Science Foundation to study nine emerging areas of research. The CEHC was the first selected to highlight emerging neuroscience research. The Center was featured on several national broadcasts and was received very well by the “Living on Earth” radio audience. This series was completed largely during this past year.
The CEHC has continued to play an important role in training researchers and clinicians in children’s environmental health. Center Director Dr. Bruce Lanphear and two of our pediatricians, Dr. Stephen Wilson and Dr. Adam Spanier, who currently are in the Ambulatory Pediatric Association’s (APA) Pediatric Environmental Health Fellowship Training Program, attended the APA’s Second Annual Environmental Health Scholars retreat in March 2004. Both Drs. Wilson and Spanier presented their research and benefited from an open discussion of their work from peers and invited faculty. Dr. Gwen Collman was an invited speaker.
A Scientific Advisory Review of the CEHC was conducted on June 2, 2003. Dr. Brenda Eskenazi, Professor of Maternal and Child Health and Epidemiology, and Director of the Center for Children’s Environmental Health Research at the University of California School of Public Health in Berkeley, California, reviewed our program. The Center was given an extremely positive review, which is available upon request.
Both the CEHC Internal Advisory Board and External Scientific Advisory Board met during this reporting period. The agenda for both meetings consisted of an overview of the Center, which included an update on projects that are part of the overall effort of Center faculty. Each project funded by the Center grant was presented and updated by each respective principal investigator or project director. Lastly, new children’s environmental health research projects were presented. Time for questions and feedback from the Board members, as well as a group discussion, completed each meeting. In addition to hearing the progress of each grant project, the focus of both Board meetings was an open discussion and exchange of ideas to assist in planning for future Center research studies and to reiterate our responsibility to the community to be informative and responsive regarding issues of children’s environmental health. Both the CEHC Internal and External Boards convene at approximately 18 month intervals.
As suggested in the Centers for Children’s Environmental Health and Disease Prevention Request for Applications, the CEHC’s original application requested funding to support two new investigators. One of those individuals, Dr. Kim Cecil, in addition to being a new scientist, is principal investigator of project R829389C005, “MR [magnetic resonance] Assessment of Brain Function Altered by Lead Exposure.”
Our other new scientist is Dr. Kim Yolton. During her first year with the Cincinnati CEHC, she spent 30 percent of her research effort on activities related to Project R829389C001, the Health Outcomes and Measures of the Environment (HOME) Study. She has been involved in questionnaire development, staff training and preparation for study visits, and discussions of future directions of the project. In addition, she has led data collection procedures related to her particular interest in how newborns may exhibit signs of prenatal exposure to environmental neurotoxicants. She has administered newborn neurobehavioral examinations on 89 of 99 (90%) HOME Study newborns shortly after birth and 73 of 82 (89%) HOME Study infants at 4 weeks of age. These efforts have resulted in preliminary data to support a proposal that was submitted on June 1, 2004, to obtain federal funding to further this study.
In the past year, she has collaborated on four manuscripts that were submitted for publication in peer-reviewed journals, one as first author. Two of these publications are in press, and two are currently in review. Other activities have included four local and one out-of-state presentation on topics related to children’s environmental health.
Dr. Yolton recently received federal funding to explore the effects of ETS exposure on the behaviors and sleep patterns of children (1 R21 ES012952-01), and she has submitted an additional grant proposal to study the impact of prenatal exposure to neurotoxicants on newborn neurobehavior as mentioned above.
Dr. Yolton’s goals for the coming year include continued collection of newborn neurobehavioral data, training and development efforts with staff related to psychometric testing, contributions to future grant proposals related to the HOME Study, execution of the grant related to ETS effects on child behavior and sleep, and exploration of additional funding opportunities in which to study further the impact of prenatal environmental neurotoxicant exposure on newborns.
