Abstract | Diamond Shamrock Corporation produces methylene chloride and chloroform at its Belle, West Virginia facility. During an upset situation, the nondistilled intermediate mixture product was found to contain noncharacteristic solid materials. The solid materials resulted in the formation of a plug in the bottom line of the chloroform distillation column. Two employees of Diamond Shamrock attempted to remove the blockage with pressurized steam. When the plug dislodged, a release occurred resulting in exposure of the two employees to an airborne mixture of chemicals. The plant was then shut down and an investigation was conducted to determine the cause of the upset and the composition of the contaminants present in the process stream. |
