Abstract The National Cancer Institute (NCI), the Environmental Protection Agency (EPA), and the National Institute of Environmental Health Sciences (NIEHS) have planned a long-term prospective epidemiologic study of men, women, and dependent children in agricultural areas to identify and quantify cancer risks that may be associated with pesticide usage. The paper covers the types of questionnaires that are to be used in the study. The main topic is the EPA developed questionnaire that was designed to assess total pesticide exposure to the farm family. The EPA developed a questionnaire to assess previous pesticide use and practices, exposure while pesticides are being applied, and any residue that may find its way into the home. The questionnaire was administered to 3 farm families during the spring of 1992. Activity logs were kept by participants during the one week study period. A food frequency log was kept of all food prepared during the day of application and information was gathered on the types of food used by the families, both locally produced and purchased from retail stores. Problems encountered during the administration of the questionnaire and steps taken to remedy the problems will be discussed.
