The Appendix is in four parts. Part I contains the general discussion of the Industrial Facilities Evaluation Model which was developed to provide the industrial costs for wastewater treatment. In part II, there is presented the detailed output from the industrial model along with some of the basic water use/employee information developed for the model. Part III contains the logic and method of the pollution index along with the summary results by second order watersheds and EPA administrative regions. Last, in Part IV, the procedure and questionnaire used in the 1971 assessment of planned construction of municipal waste treatment facilities.
