Objective:
The objectives of this project are to: (1) examine the contribution of multilevel health disparities environmental factors (HDEF) to diabetes and depression in a prospective cohort study, the Northern Manhattan Study of Metabolism and Mental Health (NOMEM); and (2) examine whether and how multilevel HDEF modify the response to community-based interventions in three randomized controlled trials—The Northern Manhattan Community Outreach Project (NOCHOP), The Northern Manhattan Caregiver Intervention Project (NOCIP), and The Counseling Adults to Control Hypertension Study (COACH). These projects are component projects of the Northern Manhattan Center of Excellence for Minority Health and Health Disparities (NOCEMHD) at Columbia University.
Progress Summary:
Recruitment for the Environmental Health Disparities Supplement (EHDS) from the four projects described in the application has been completed for three of the studies (NOCHOP, NOCIP, COACH) and is ongoing for NOMEM. The final enrollment for NOCHOP was finished on 02/28/13 and it was 225 participants. The final enrollment for COACH was finished on 10/30/12 and it was 149. The final enrollment for NOCIP was completed on 06/30/13 and it was 105 participants. As of 07/10/13 the enrollment in NOMEM is 355 participants. Dr. March has begun (1) the geocoding and linkage of residential addresses of participants to link to census data; and (2) the analysis of local, city, and state social and environmental policies that may bear on disparities. In our last progress report, we reported that we submitted a grant that explicitly builds on the resources generated by this project; we submitted an application entitled “New York City Hispanic Dementia Caregiver Research Program (NHiRP)" (1R01NR014430-01) in response to RFA-NR-13-001 “Addressing Needs of Informal Caregivers of Individuals with Alzheimer’s Disease in the Context of Socio-demographic Factors (R01)." We are glad to report that this grant was funded as of 06/01/2013. This will continue the work of examining environmental and social stressors in relation to health outcomes among Hispanic caregivers of persons with dementia related to NOCIP and in a new registry of Hispanic dementia caregivers that will be recruited. We also submitted the grant “BASES of Pre-Diabetes and Diabetes in Middle Aged Hispanics" (1R01MD007782-01), which also sought to continue longitudinal data collection of the middle aged cohort (NOMEM) that is part of the EPA grant. However, this grant received a score of 3.5 and was not funded.
Future Activities:
- A review paper on the literature addressing environmental contextual factors has been completed and should be submitted for publication this month.
- We will submit a paper analyzing baseline data for NOCHOP by 08/31/13 followed by the COACH baseline data analyses.
- The NOCIP data collection was recently completed, and we are cleaning the data for analysis.
- Drs. Luchsinger and March will attend the Environmental Health Disparities and Environmental Justice meeting in North Carolina July 29 to 31, 2013.
- Data collection for NOMEM will finish on 12/31/13.
- We anticipate that the main NOCHOP, COACH, and NOCIP manuscripts examining the effect of HDEF on the interventions in these trials will have to wait for submission until the end of this year, when the parent study manuscripts are expected to be accepted for publication.
- The rich data from NOCHOP, COACH, NOCIP, and NOMEM will be part of a Data and Biospecimen Repository that will be maintained by the parent P60 Center of Excellence for secondary data analyses, ancillary studies, and training and career development activities from inside and outside the institution. We expect that the volume of analyses and manuscripts developed and submitted will reach its peak in the 2014 calendar year.
