Objective:

The long-term goal of the ASTHMA-DIET (A Study To Understand The Mechanisms Of Asthma--Dietary Interventions To Protect Against Environmental Triggers) Program is to understand how diet influences the asthmatic response to indoor and outdoor airborne pollutants and allergens, with the expectation of translating these findings into practical dietary strategies to improve pediatric asthma health. The ASTHMA-DIET Center’s overall hypothesis is that a low anti-oxidant, pro-inflammatory diet impairs the capacity to respond to oxidative stressors, thereby increasing susceptibility to pollutant and allergen exposures. The aims of the Center proposal are focused on the mechanistic underpinnings of airway inflammation, oxidative stress and respiratory morbidity in predominantly African-American children with asthma, living in the inner city of Baltimore.
 
The specific aims are to:  (1) examine the effect of the poor “inner city” diet on susceptibility of asthmatic children to in-home pollutant (PM and NO2) exposure; (2) identify barriers and enabling factors to implementing dietary and environmental modifications in inner city homes of children with asthma; (3) conduct randomized, controlled trials of two dietary interventions: namely broccoli sprouts (BS), which increase OS capacity in respiratory epithelium, and the low-saturated fat, anti-inflammatory OmniHeart Diet (OHD); (4) test the effects of BS and the OHD on OS, inflammation, basophil activation, and clinical outcomes in Baltimore City adults with mouse allergen-induced asthma; (5) determine the effect of dietary constituents (proposed in Project 2-children intervention studies) on OVA and ambient particle induced-asthma in mice models. The mice will be fed either a normal diet or a modified Mediterranean-type diet from which we will compare diets with low DHA, high DHA and a high fat diet; (6) study asthma susceptibility by measuring different endpoints such as pulmonary inflammation, Th2 cytokines, and airway hyper-responsiveness; (7) study the regulation of the Nrf2 pathway and its downstream antioxidant gene activation due to sulforaphane using Nrf2 +/+ and -/- mice to dissect the mechanism; and (8) determine the effect of dietary constituents on DC maturation and Th2 polarization in the OVA and the ambient particle model of asthma. The proposed studies will help illustrate the mechanism by which diet modulates susceptibility and asthma pathogenesis.

Progress Summary:

1.     In the last 24 months of the program award, we have made significant progress to integrate all components of the program, ensure tight collaboration between the leaders of the three projects and four cores, and exchange of ideas and data. Project 1 began recruitment of study subjects last year and 91 of the planned 200 subjects have been enrolled. While the study requires 1 year of enrollment, 19 subjects have completed the entire study. In Project 2, substantial development work was completed on determining the best quantity and packaging for the broccoli sprout intervention. Planning for the OmniHeart intervention is underway.  In Project 3, planned animal studies are either completed or underway. For Specific Aim 1, the broccoli sprout intervention is complete and was successful at protecting against an allergic asthmatic phenotype in the mice. The results are being compiled for submission to a peer review journal.
 
Investigators from each Project and Core have been meeting weekly at regularly scheduled staff and data meetings, with informal meetings even more frequently. The Cores are all fully functional and productive. Core A has been instrumental in organizing a multi-investigator scientific retreat at the Mt. Washington Conference Center. Core B has embarked on many tasks, including forms development, database development and refinement, sample tracking and upgrade of a data web portal. Core B has been actively entering and storing data for all projects. Core C had developed exposure assessment protocols and is actively engaged in collection of environmental data. Core D has convened our Community Advisory Board and is actively working with the CAB members and Head Start to conduct Aim 2 of Project 1.
 
2.     There has been significant progress on project 1 in the past year. The field staff was trained in the study procedures and training continued in this period including weekly recruitment meetings to monitor progress, performing quality control of the collected data and strategizing recruitment strategies.
 
Among 91 participants enrolled at this point, 19 participants have completed the study, 26 have completed two seasons and 46 participants have completed home visits for the first season. At baseline visit, health information was collected and lung function test was done. On the following visits, questionnaires, diet survey, home inspections, exhaled nitric oxide, skin assessments, peak flow measurements, dust sample collection and air monitoring was done. Air monitoring includes PM2.5, PM10, NO2 measurements and samples were sent to the environmental assessment core. Blood, urine and nasal lavage samples have been collected for all the participants who have completed at least one season and delivered to Dr. Biswal’s laboratory for the processing to measure inflammatory and oxidative stress markers. Blood samples have also been delivered to Dr. Detrick’s laboratory to measure serum 25-hydroxyvitamin D levels. On the initial samples run, the mean vitamin D level is 18.9 +/- 8 ng/mL. This study will fill a significant gap between the evidence base for the effect of diet on susceptibility to indoor air pollutants on asthma health. To our knowledge, the proposed study will be the first to examine the interactive effect of diet with indoor pollutants (PM and NO2). The results of this study will help to establish the role and mechanism of effect of diet and indoor pollutants on inner city childhood asthma, and lay the groundwork for intervention treatments and strategies.
 
3.     Significant progress has been made on Project 2. A study coordinator has been hired for the project and protocol, consent forms, manual of procedures, and data collection forms developed and finalized for the Sprouts Study. In addition, substantial work has been done in collaboration with the Johns Hopkins University ICTR Nutrition Core to develop a palatable means of ingesting the broccoli sprouts and a means of blinding participants to the sprouts content of the sandwiches (alfalfa vs. sprouts). A pilot tolerability study was launched and completed to identify the maximal broccoli sprouts dose that causes minimal to no gastrointestinal complaints. IRB and ICTR approval have been obtained. In addition, pilot studies for oxidative stress biomarker and gene expression assays have been completed so that these assays are ready to be implemented in the planned Sprouts and Omni Heart Diet studies. Additional progress is detailed below:
 
  • Phase 2 Enzyme Gene Expression Pilot Studies
We have conducted pilot studies measuring Phase II gene expression in nasal epithelial cells from several study populations. Methods for collection of nasal epithelial cells have been piloted, and we have established the feasibility of collecting the samples and of extracting sufficient quantity and quality RNA for gene expression assays. For purposes of our pilot study, nasal epithelial cell samples were collected from an ongoing cohort of Baltimore City children with asthma and levels of Phase II gene expression were compared to markers of asthma severity. Findings from this pilot study were published in the Journal of Allergy and Clinical Immunology in 2011.
 
  • Pilot Tolerability Study
A pilot study examining tolerability of 100 g and 75 g broccoli sprouts (BS) delivered in sandwiches was completed. Five healthy adults were enrolled for the 100g dose and five enrolled for the 75 g dose pilot. Participants consumed sandwiches daily for 5 days. Participants reported that the sandwiches were very palatable and consistently consumed at least 85% of the sandwich at each meal. Plasma sulforaphane (SFN) concentrations before ingestion were undetectable and 1 hour after ingesting the last sandwich, sulforaphane concentrations were detectable in the 600-2290 nM range. The only side effects were gastrointestinal with bloating and abdominal pain reported most commonly. There was no vomiting or diarrhea reported. There were no differences in side effects between the 75 g and 100 g BS sandwiches. There were no significant changes in thyroid function tests associated with ingestion of BS sandwiches. The 100 g broccoli sprout sandwiches were associated with somewhat higher plasma SFN concentrations and were selected for the efficacy trial. Results of this pilot study were presented at the 2011 meeting of the American Thoracic Society.
 
  • Sprouts Study
A crossover trial of BS vs. alfalfa sprouts sandwich was launched in 2010. The primary outcome was a drop in FEV1 in an environmental mouse allergen chamber inhalation challenge. Currently, a revised protocol is under development with the plan for conducting nasal allergen challenges instead of environmental allergen chamber inhalation challenges.
 
  • Omni Heart Diet Study
In preparation for this study of a dietary intervention that replaces the urban diet with a Mediterranean-like diet, we are preparing the IRB application for the Omni Heart Diet Study. We have been meeting with the research nutrition group at the ICTR to refine our protocol for delivering the dietary intervention. We will use the Food Frequency Questionnaire that is currently working well in the panel study outlined in Project 1 to assess characteristics of the participants’ baseline dietary intake. We have incorporated the experience that we have gained through the nasal epithelial cell gene expression pilot studies in refining the protocol for collection of outcome measures.  We plan to recruit using a similar strategy as that for the Sprouts Study.
 
These studies will provide important insight into: (1) the mechanisms by which dietary factors may promote or protect against environmentally induced airway inflammation; and (2) potential dietary interventions for asthma and allergies aimed at providing protection against allergen-induced asthma.
 
4.     The effect of Sulforaphane, the main ingredient in broccoli sprouts is complete. Sulforaphane protects from developing OVA-induced experimental asthma in mice. It inhibits cardinal features of asthma like airway inflammation, cytokine expression and AHR in mice. Further studies of the biochemical mechanisms underlying the observed activities is in progress. The preliminary study on antioxidant-rich Mediterranean-type diet in our mouse model of asthma is in progress. Studies of further biochemical mechanisms underlying the observed activities is in progress.
 
We have developed and characterized Nrf2 & Keap knockout mice by crossing of Nrf2/ keap flox/flox mice with Tamoxifen inducible CMV-CRE mice). Deletion of Keap1 by tamoxifen protects the OVA-induced asthmatic features like airway inflammation, cytokines expression and AHR in mice. Now, we are studying the effect of Nrf2 deletion induced by Tamoxifen. These studies are geared towards understanding the Nrf2 pathway as a critical modifier of pulmonary oxidative stress and asthma. Nrf2 activators could be the lead molecules for therapeutic interventions.
 
5.     In addition to maintenance of the data management system, the Data Management and Statistics Core (DMSC) has provided data support services for all projects. The DMSC has developed forms, study participant recruitment and tracking systems, provided interim and final analyses of accumulated data, and participated in the preparation of scientific reports and manuscripts.
 
The DMSC holds weekly meetings with investigators and research staff of the Center for Childhood Asthma in the Urban Environment to present reports of interim analyses, troubleshoot data collection or quality issues, and this meeting also provides a forum for discussions regarding abstract and manuscript preparation. In addition to the weekly meetings, the following objectives have been met:
  • Completion of a Tracking/Recruiting Database for Project 1; Tracking/Recruiting Database for one Project 2 study is completed and for the other study, under development.
  • Completion of Data Collection Forms for Projects 1 and 2.
  • Identification of potential study participants from the Future Subjects Database for Projects 1 and 2.
  • Upgrade of the user interface and capabilities of the Environmental Data Web Portal.
  • Expansion of a Web-based data portal to manage data generated in the Exposure Assessment Core, including PM, NO2, airborne nicotine, HOBO temperature and humidity, and MIE data.
  • Implementation of a bar coding system to track biologic samples.
  • Maintenance of a database of potential study participants (Future Subjects Database) to be used as a recruitment tool.

The following projects are supported by the DMSC, but are not directly related to the CCAUE:

  • Data analyses and manuscript preparation for the MAACs study, an observational cohort study of urban children with asthma.
  • Form development, database development, and data analyses for several food allergy studies.
  • Database development, data management, and data analyses for the JAXCohort study, a prospective cohort study of newly employed laboratory animal workers.
  • Database development, data management, and data analyses for the Mouse Allergen and Asthma Intervention Trial (MAAIT), a randomized controlled trial of mouse allergen abatement in mouse allergic children with asthma.
  • Environmental data management and data analyses for the CODE study, an NIH-funded study of airborne and settled dust endotoxin and COPD.
  • Forms and database development, environmental data management, and data analyses for COPD-PM, a study investigating the impact of indoor particulate matter exposure on COPD.
  • Database development, data management and data analyses for the STOVE study, a HUD-funded study testing the effects of housing interventions on indoor NO2 levels.
These completed and in progress projects are in addition to the day-to-day work of form development, database construction, data entry, data cleaning, quality control checks, preparation of reports, and statistical analysis.
 
The research projects supported by the Data Management and Statistics Core will result in important insights into the impact of diet on asthma, including whether diet interacts with PM and allergen exposure to cause greater asthma symptoms and morbidity, and whether diet interacts with PM or allergen induced symptoms via the oxidative stress pathway. Ultimately, these findings will provide the foundation and rationale for dietary recommendations for asthmatics as well as a rationale for the use of dietary interventions as therapy in the management of asthma.
 
6.     Progressing with the specific aims of the Environmental Assessment Core, in year 2 we have collaborated in the development and conduct of the exposure analysis component of the clinically-based and mechanistically based projects. This has included developing exposure assessment protocols for Projects 1 and 2. For project 1, the homes of 46 study children with asthma have been sampled for PM and NO2. Results are being posted to the intranet website to allow investigators access to findings as the measurements are obtained. For project 2, air samples were collected for 32 participants in the mouse challenge chamber. On an ongoing basis, we collaborate with the Data Management Core on the development and implementation of exposure assessment tools, forms and databases. The Core PI attends all Community Advisory Board meetings and participates in planning for Community Outreach and Research Translation Core activities.
 
7.     The Communitiy Outreach/Translation Core continues (COTC) to meet with our Community Advisory Board (CAB) on a regular basis to discuss the overall goals and progress of the “Mechanisms of Asthma—Dietary Interventions against Environmental Triggers” projects, and solicit their advice on project implementation and strategies for sharing new information with the community. The agendas for our CAB meetings focus on soliciting CAB suggestions for strategies that we might use to assess community concerns, issues, barriers and resources related to environmental exposures, diet and asthma. We have specifically sought CAB input in identifying key themes and issues that our low-income community families and children regularly face in obtaining and preparing healthy. We have used this input to help shape measures of dietary barriers for Project 1, Aim 2. We have met with our Community Outreach/Translational Core co-investigator Joel Gittelsohn, Ph.D. (whose expertise is in diet in inner city communities), to adapt questions from his inner city dietary questionnaires, as appropriate, for the Asthma-Diet home-based family surveys. In follow-up to recommendations from our CAB, we have designed a pilot qualitative study of barriers and facilitators of healthy eating in inner city families of young children to be conducted with Head Start families. We have obtained Head Start permission to conduct this study, IRB approval for this study, and have identified and trained an appropriate staff person to recruit and conduct focus groups for this project. We will begin enrolling participants into these focus groups in early fall 2011.
 
We continue to serve as a local community resource for outreach and dissemination of information about asthma and the environment. One our key community partners for outreach and dissemination is Head Start of Baltimore, which serves high-risk, low-income preschool children. We have ongoing educational activities related to children’s environmental health that we conduct in partnership with Head Start. For example, in the last project year we have sponsored and provided materials and health education support to multiple Head Start sites related to reducing children’s exposure to environmental tobacco smoke in the home. We have sponsored “Smoke-Free Head Start Family” days at several Head Start sites. Our asthma educator regularly offers asthma education training sessions for Head Start teachers and staff and participates in community and Head Start community health fairs to distribution information about asthma and respiratory environmental health. We will continue these community education activities in year 3, along with our regular meetings with our CAB.

Future Activities:

This study will fill a significant gap between the evidence base for the effect of diet on susceptibility to indoor air pollutants on asthma health. To our knowledge, the proposed study will be the first to examine the interactive effect of diet with indoor pollutants (PM and NO2). The results of this study will help to establish the role and mechanism of effect of diet and indoor pollutants on inner city childhood asthma, and lay the groundwork for intervention treatments and strategies.
 
The plans for year 3 include accrual and data collection, to obtain approvals for the re-launch of the sprouts study, and to obtain regulatory approvals for and launch the Omni Heart Diet intervention study.
 
Our studies are geared towards understanding the Nrf2 pathway as a critical modifier of pulmonary oxidative stress and asthma. Nrf2 activators could be the lead molecules for therapeutic interventions. We aim to define the role of diet in asthma pathogenesis and would like to explore Nrf2 as modifier of the disease pathogenesis. We have projected diet-based Nrf2 activators in preclinical studies for developing Nrf2 activator as a therapeutic lead molecule in asthma. We plan to
  • Expand the Environmental Data Management Website to improve functionality of current features and expand capabilities.
  • Conduct preliminary analyses for Projects 1 and 2.
  • Interface with Project 3 to collect assay data and merge assay data with Projects 1 and 2 clinical and environmental datasets.
  • Maintain and improve Future Subjects Database, the database of children and adults who have participated in previous studies who may be contacted for participation in future studies.
  • Our asthma educator regularly offers asthma education training sessions for Head Start teachers and staff and participates in community and Head Start community health fairs to distribution information about asthma and respiratory environmental health. We will continue these community education activities in year 3, along with our regular meetings with our CAB.
