Objective:

The Multi-Ethnic Study of Atherosclerosis and Air Pollution (“MESA Air”) was designed to provide the most advanced approach possible to define the relation between air pollution and cardiovascular disease (CVD). MESA Air is the first fully prospective study of this relationship, and effectively merges state-of-the-art cardiovascular epidemiology, including repeated measures of subclinical disease, with cutting-edge exposure assessment, including comprehensive, study-specific air pollution monitoring and novel exposure modeling.

Specifically, MESA Air aimed to prospectively examine the relation between individual-level assessment of long-term air pollution exposures (including PM2.5 and gaseous co-pollutants) and the progression of subclinical cardiovascular disease in a multi-ethnic cohort, by repeatedly assessing coronary artery calcification (CAC) and intima-media thickness (IMT) of the carotid artery. The project was also intended to further our understanding of the biological mechanisms underlying any observed air pollution effect by examining changes over time in plasma markers of inflammation, endothelial activation, coagulation and fibrinolysis. We also aimed to assess the relation between long-term air pollution exposures and incidence of cardiovascular disease events, including myocardial infarction and cardiovascular disease mortality. A further objective was to identify whether specific populations are especially susceptible to the effects of air pollution, by evaluating the potential for factors such as race/ethnicity, age, socioeconomic status (SES), and specific cardiovascular risk factors (such as diabetes, hypertension, smoking and diet) to modify any observed relationship between air pollutants and clinical or subclinical disease. We also aimed to assess the joint and independent effects of acute and long-term exposures to air pollutants in the occurrence of cardiovascular disease events and levels of the measured plasma biomarkers.

To investigate these relationships, MESA Air was designed to assess individual-level exposure to PM2.5 , NOX, and NO2 of ambient origin using community-scale monitoring, outdoor spatial variation, subject proximity to sources such as highways, residential characteristics including pollutant infiltration efficiencies, and personal time activity information. These community-scale air measurements involved multiple air pollution monitoring campaigns, including campaigns with extensive spatial coverage and others with long temporal coverage. We then aimed to integrate all available data sources using sophisticated spatio-temporal exposure models, able to provide participant-specific air pollution exposure estimates on flexible time scales. Another objective was to assess the relation between air pollutants and cardiovascular disease using alternate exposure measures focusing on chemical composition of particles (including trace elements via x-ray fluorescence and black carbon). As part of the work described above, we intended to refine existing statistical methods to ensure robustness of analyses to address spatial clustering, and to help define the contribution of individual pollutants, particulate chemical components, and particulate sources to the development of clinical and subclinical cardiovascular disease.

Summary/Accomplishments (Outputs/Outcomes):

MESA Air successfully developed individual-level exposure estimates to outdoor ambient-source air pollutants for 7,750 MESA and MESA Air participants, using state-of the-art exposure assessment methodologies. These exposure estimates incorporated data from several sources including: extensive, project-specific air monitoring programs within the study communities and at participants’ homes involving the deployment of over 7,400 monitors collecting cumulative, time-integrated data over two-week segments throughout a four year period; all available Air Quality System (AQS) monitoring data in the relevant communities; and a custom-built, comprehensive database of more than 300 geographic and traffic-related variables providing information such as subject residential proximity to potential exposure sources. These data sources were integrated using a unified spatio-temporal exposure model to estimate the following ambient-source pollutant concentrations at each participant’s residential location: total mass of airborne particles with aerodynamic diameters less than 2.5 micrometers (PM2.5 ), oxides of nitrogen (NOX), nitrogen dioxide (NO2) and black carbon (BC). Estimates were generated for every two week period from the time of recruitment over the entire 10-year study period. These exposure models were region-specific and included a long-term spatial mean, temporal trends with spatially varying coefficients, and a spatiotemporal residual. Prediction accuracy was high for most models, with cross-validation R2 greater than 0.80 at regulatory and fixed monitoring sites for most regions and pollutants.

In addition to estimating outdoor concentrations, we successfully employed individual-level, questionnaire-derived data on residential characteristics to calculate particulate matter infiltration efficiencies for all of the residences in which each participant lived during the MESA Air study period. These models predicted 60% of the variance in 2-week residential infiltration efficiency estimates. These models were then used to estimate indoor concentrations of ambient-source pollution. Using participant-specific time-location information, also ascertained via questionnaire, we calculated individual-level exposures, time-weighted across indoor and outdoor environments.

Using these exposure estimates, MESA Air was able to thoroughly examine, in a prospective manner, the relation between air pollution and subclinical cardiovascular disease. We examined the relationship between outdoor residential pollutant concentrations and individual-level exposures to ambient-source pollutants and the subclinical progression of atherosclerosis by repeated measurements of CAC measured via computed tomography and IMT measured via carotid ultrasound. We used an innovative application of mixed models which provided a statistical procedure to adjust the longitudinal associations for the cross-sectional air pollution relationships observed at baseline in an unbiased manner. Both outcomes were assessed in staged adjustment models including essential demographics (age, sex and race/ethnicity), indicators of individual-level and neighborhood-level socioeconomic status (SES), statin use, and other important cardiovascular risk factors such as family history of premature cardiovascular disease, adiposity, cholesterol, physical activity, smoking status, and second-hand smoke exposure. All models also included metropolitan area to account for the stratified nature of the study and clinic-specific effects.

We found that outdoor concentrations of PM2.5 and NOX were associated with accelerated CAC progression rate over 6.2 mean years of follow-up. A concentration of 5 μg/m3 higher PM2.5 was associated with a 4.1 Agatston unit per year increase (95% CI: 1.4, 6.8) in CAC progression. A 40 ppb higher NOX concentration was associated with a 4.8 Agatston unit per year increase (95% CI: 0.9, 8.7) in CAC progression. We also investigated whether the observed relationships were modified by demographic or socioeconomic characteristics or by cardiovascular risk factors. Neither sex nor race/ethnicity nor obesity was found to be a significant modifier of the air pollution effect. However, we did observe that individuals with diabetes or hypertension appear to be more sensitive to the effects of air pollution than those without these diseases. We also observed evidence that the effect of air pollution increases with age.

Positive but non-significant associations were observed between CAC progression and outdoor concentrations of NO2 at participant residences (2.7 Agatston units/year for an additional 10 ppb NO2, 95% CI: -0.3, 5.7) as well as individually-weighted, ambient-derived PM2.5 exposure (1.3 Agatston units/year for a 5 μg/m3 higher exposure, 95% CI: -1.7, 4.3). IMT progression, a measure with less precision and in which we had fewer longitudinal observations for analysis, was not associated with any of the air pollution exposure estimates.

We examined the association of long-term air pollution and two markers of inflammation: C-reactive protein (CRP) and interleukin-6 (IL-6); two markers of coagulation and fibrinolysis: D-dimer and fibrinogen; and two markers of endothelial activation: soluble E-selectin and soluble Intercellular Adhesion Molecule-1 (sICAM-1). Using a repeated measures design and a random effects modeling approach, we found that exposure to 5 μg/m3 higher individual-level, ambient-derived PM2.5 concentration (integrating estimated, residence-specific indoor concentrations and person-specific time-location information) was associated with 5.5% higher IL-6 levels (95% CI: 1.83, 9.36) and a 40 ppb higher outdoor residential NOX concentration was associated with 7.1% higher D-dimer levels (CI: 1.62, 12.83) after control for relevant confounders. In addition to these primary findings, we found suggestive evidence that older individuals, smokers and those with hypertension experienced larger pollution-related increases in IL-6 and D-dimer compared to younger, nonsmoking, normotensive participants.

We further explored the independent and joint effects of acute and long-term exposure to air pollutants on these plasma biomarkers. Short-term concentrations of PM2.5 reflected several averaging periods prior to the date of blood draw, including: day of blood draw, prior day, and moving average of prior 2, 3, 4 and 5 days. A 5 μg/mL increase in PM2.5 on the day of the blood draw was associated with a 0.60 μg/mL increase in E-selectin (95% CI: 0.06, 1.14) and there was evidence suggestive of a positive association with CRP and fibrinogen (CRP: 1.49%, 95% CI: -0.34, 3.35; fibrinogen: 1.16 mg/dL, 95% CI: -0.28, 2.61). Furthermore, an examination of the joint effects of acute and long-term exposures showed that associations of interest in the long-term analysis were not substantially altered with the inclusion of short-term PM2.5 exposures, suggesting that these associations were not being driven by short-term PM2.5.

An additional primary aim of MESA Air was to assess the association between long-term air pollution exposures and incidence of CVD events using the highly spatially- and temporally-resolved exposures previously described, along with the gold standard events adjudication that is a hallmark of modern epidemiological cohorts such as MESA. Analyses focused on a composite CVD endpoint (“All CVD”) as well as specific endpoints such as myocardial infarction (MI), coronary heart disease (CHD), stroke and congestive heart failure (CHF), and used long-term air pollutant concentrations calculated at various temporal scales to assess the degree to which duration of exposure might affect incidence. Initial power calculations anticipated 80% power to detect a 4% increase in the risk of CVD events per 1 µg/m3 change in PM2.5 over the 10 years of follow up. However, events accrual in MESA has been slower than expected. Based on initial projections, we anticipated 1,020 events by the end of 2011, and nearly 1,200 by the end of 2013. Instead, the annual event rate for this combined endpoint has been less than the expected rate of 1.5% throughout the course of the study, such that the actual number of adjudicated events by the end of 2011 was only 662. As such, results from these analyses are unfortunately underpowered to confidently determine effects of the hypothesized magnitude; results from events analysis should therefore be considered preliminary.

Despite the lower than expected accrual rate, we nonetheless found significant associations between risk of “All CVD” events and PM2.5 exposure in the two years prior to the event (HR: 1.44 per 5 μg/m3, 95% CI: 1.07-1.94). We also observed elevated risk of “All CVD” events associated with exposure to NOX on this time scale (HR: 1.25 per 40 ppb, 95% CI: 0.89-1.76), although this association was not statistically significant. Associations between long-term exposures to air pollutants and MI and stroke were suggestive of elevated risk for both PM2.5 and NOX, but were not statistically significant. For all endpoints with the exception of CHF, exposures averaged over the two years prior to the event were more strongly associated with CVD risk than those reflecting cumulative exposure since enrollment.

Although MESA Air focused primarily on exposures to particulate mass, understanding the relation between cardiovascular disease and specific chemical constituents has the potential to illuminate causative agents, as differences in chemical composition and sources of air pollution may result in differential effects on cardiovascular health. To investigate the effect of differential sources and components of particulate air pollution on health, all MESA Air samples analyzed for PM2.5 concentrations were also analyzed for black carbon (BC) as measured by light absorption coefficient. Further, in conjunction with one of its ancillary studies, the Health Effects Institute’s National Particle Component Toxicity (NPACT) Initiative, these samples were analyzed for trace elements by x-ray fluorescence and a subset of samples were analyzed for organic carbon (OC) and elemental carbon (EC). MESA Air and NPACT investigators evaluated the cross-sectional relationship between CAC and IMT measurements and several components of particulate matter, and observed evidence that carotid IMT was associated with OC, sulfur and silicon.

Conclusions:

MESA Air successfully integrated data from an extensive, community-centered air monitoring effort with all available Air Quality System monitoring data and a comprehensive, custom database of geographic and traffic-related covariates into state-of-the-art spatio-temporal exposure models. These models are capable of estimating outdoor concentrations of ambient-source pollutants through the MESA Air study regions with cross-validated R2s above 0.80 at most regulatory and fixed site locations. This flexible and powerful modeling approach can incorporate an unbalanced monitoring data structure, and is easily transferrable between regions and pollutants. While aspects of the MESA Air exposure modeling techniques are particularly tailored to the unique data and resources of this study, lessons learned here can be applied to future cohort studies in understanding the spatial and temporal variation of pollutants. Advances in fine-scale modeling resolved in both space and time are important for the next generation of cohort studies assessing health effects of environmental agents.

Employing these state-of-the-art exposure estimates, MESA Air has determined that long-term exposures to PM2.5 and NOX are associated with increased levels of plasma markers of inflammation and coagulation and with an acceleration of subclinical progression of atherosclerosis as indicated by repeated measurement of coronary artery calcium. Older participants and those with specific cardiovascular risk factors such as smoking, diabetes and hypertension were found to be especially susceptible to these effects. Analysis of the independent and joint effects of acute and long-term exposures to air pollution on the plasma biomarkers indicated that these associations were not driven by short-term PM2.5 exposures.

In addition to the subclinical findings, the results of this study also suggest that long-term exposures to air pollutants including PM2.5 and gaseous co-pollutants are associated with increased incidence of a composite measure of CVD events, including myocardial infarction, resuscitated cardiac arrest, definite or probable angina, stroke or stroke death, coronary heart disease death, and other atherosclerotic/CVD death. However, these results were unfortunately underpowered; we expect that additional accrual of events will serve to strengthen the statistical evidence of these findings and result in the reduction of uncertainty desired from this study.
