Objective:

The goal of the Multi-Ethnic Study of Atherosclerosis and Air Pollution Study (MESA Air) is to prospectively examine the relation between an individual level assessment of long-term ambient air pollution exposure (including PM2.5 and gaseous co-pollutants) and the progression of subclinical cardiovascular disease in a multi-city, multi-ethnic cohort.  MESA Air also will prospectively examine the relation between an individual level assessment of long-term ambient air pollution exposures and the incidence of cardiovascular disease, including myocardial infarction and cardiovascular death.

Progress Summary:

During the sixth year of MESA Air, the research team has:

  • Completed sample analysis from the exposure monitoring component of the project, and followed rigorous quality control procedures for data cleaning.  Resulting datasets met data quality objectives specified in the QAPP.
  • Developed (and published) a pragmatic spatio-temporal approach to modeling ambient PM2.5 concentrations across the MESA Air study regions from 2000-2009 using combined regulatory and MESA Air monitoring data.
  • Developed initial predictions of long-term average concentrations of NOx during a 10-year period in Los Angeles and Baltimore, using a likelihood-based spatio-temporal model incorporating dispersion model outputs. An output of this work was the development of an R-code package to produce these estimates.
  • Developed a customized and automated process for the production of CALINE dispersion model estimates, eliminating manual entry, reducing run times and improving output. 
  • Finalized all questionnaires and protocols for Exam 5, including the CT and ultrasound scanning protocols and revisions to the Air Questionnaire.
  • Conducted in-person trainings for the sonographers and for the MESA interviewers who administer the Air Questionnaire. 
  • Finalized selection criteria for the high-tech tests and put into place electronic data capture systems.
  • Began Exam 5 in April, 2010, and to date, 1338 MESA Air participants have had their clinic visit. 97% of these have completed the Air Questionnaire.  Of the 1338 who have come into the clinic, 808 were selected for the subclinical cohort, and of these, 75% have undergone the carotid ultrasound and 59% have undergone the cardiac CT scan.

Future Activities:

During the next project period, we will continue to work closely with the Field Centers and the Coordinating Center on Exam 5 activities, which will continue through the year.  By the end of the next year, we expect to have completed nearly 90% of the Exam 5 visits, and to have acquired approximately 3,200 ultrasounds and CT scans.  We also will focus significant attention on exposure modeling, and are planning to finalize likelihood-based spatio-temporal model predictions for ambient concentrations of PM2.5, NO/NO2/NOx, and LAC/EC/OC. We will finalize our infiltration models for PM2.5, allowing us to utilize the time-location data that have been collected to produce individual-level exposure estimates, one of the primary aims of this project.  Finally, we will continue to conduct early health analyses, to develop plans for ultimate health modeling strategies, and to emphasize publications.
